Job summary

Molina Healthcare is seeking a remote RN Care Manager to support Medicare members through telephonic case management and care coordination. The role is remote with a preference for Washington state, and hours align with Pacific Standard Time.

Candidates will perform assessments, develop care plans, conduct visits as needed, and collaborate with a multidisciplinary team to improve outcomes and manage costs. Travel may be required 25-40% depending on state requirements.

Qualifications

• At least 2 years experience in health care, preferably in care management, or equivalent.
• RN with active unrestricted license in state of practice.
• Valid driver's license and reliable transportation for travel.
• Understanding of EMR and HIPAA.
• Knowledge of community resources.
• Ability to work independently with minimal supervision.

Responsibilities

• Completes comprehensive assessments per timelines and determines eligibility for care management.
• Develops and implements care coordination plan with member and care team.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plan and documents interventions and progress.
• Maintains ongoing member caseload with regular outreach.
• Promotes integration of services across behavioral health and LTSS.
• Facilitates ICT meetings and collaboration.
• Uses motivational interviewing to educate and motivate change.
• Assesses barriers to care and provides resources.
• May provide consultation to peers as needed.
• May handle complex cases and medication reconciliation; travel 25–40% locally.
