Job summary

Molina Healthcare is seeking a Remote RN Care Manager to support Medicare members via telephonic case management and transition-of-care activities. The role is remote with PST hours and a Washington state preference.

You will complete assessments, develop care plans, coordinate with the care team, and monitor progress. Some travel (25–40%) may be required; prior managed care or EMR/HIPAA experience is preferred.

Qualifications

• Minimum 2 years healthcare or care management experience.
• Active RN license in state of practice.
• Valid driver’s license and reliable transportation.
• Knowledge of EMR and HIPAA.
• Ability to work independently in diverse settings.
• Strong communication and time-management skills.
• Proficiency with Microsoft Office and online portals.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Develop and implement care coordination plans with members and team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plans, document interventions, and adapt as needed.
• Maintain ongoing member caseload with regular outreach.
• Promote integration of behavioral health, LTSS, and home resources.
• Facilitate interdisciplinary care team meetings and collaboration.
• Educate and motivate change using motivational interviewing.
• Identify barriers to care and provide coordination to address concerns.
• May provide consultation and support to peers.
• May handle complex cases and medication reconciliation.
• Travel 25-40% as required.
