Molina Healthcare is seeking an RN Care Manager to support Medicare members remotely, operating on Pacific Time hours. The role emphasizes telephonic case management, care coordination, and collaboration with a multidisciplinary team to improve outcomes and cost-effectiveness.

Preferred candidates have RN licensure, 2+ years in care management, and EMR experience. Occasional travel up to 25-40% may be required depending on state requirements.

Qualifications

  • Active RN license in state of practice.
  • Minimum 2 years healthcare experience, preferably in care management.
  • Experience with EMR and HIPAA compliance.
  • Valid driver's license and reliable transportation for travel as needed.
  • Ability to work remotely across PST hours.
  • Excellent communication and problem-solving skills.

Responsabilités

  • Completes comprehensive assessments within regulated timelines to identify care management eligibility.
  • Develops and implements care coordination plans with the care team and member network.
  • Conducts telephonic, in-person, or home visits as required.
  • Monitors and documents progress, interventions, and goal achievement.
  • Maintains ongoing member caseload with regular outreach.
  • Promotes integration of behavioral health, LTSS, and community resources.
  • Facilitates ICT meetings and collaboration across teams.
  • Uses motivational interviewing to support change during contacts.
  • Assesses barriers to care and provides necessary assistance.
  • May provide consultation to peers as needed.
  • May manage complex cases and medication regimens; perform med reconciliation as needed.
