Qualifications

• Familiarity with EMR and HIPAA regulations.

Responsibilities

• Support care management and care coordination with multidisciplinary team.
• Conduct comprehensive assessments and determine care management eligibility.
• Develop and implement care coordination plans with member and care team.
• Perform telephonic, in-person, or home visits as required.
• Monitor and adjust care plans based on progress and outcomes.
• Manage an ongoing member caseload and outreach.
• Promote integration of services across care settings and resources.
• Facilitate ICT meetings and collaborative teamwork.
• Educate members using motivational interviewing and clinical guidelines.
• Identify barriers to care and coordinate needed resources.
