Job summary

Molina Healthcare is seeking a remote RN Care Manager for our Washington State Health Plan. This position is remote with a preference for candidates in Washington state and local Pacific Time coverage.

This role will support Medicare members through telephonic case management and transitions of care. Essential duties include comprehensive assessments, care coordination planning, and ongoing monitoring with potential 25-40% travel.

Qualifications

• At least 2 years in health care, care management, or equivalent experience.
• RN license active and unrestricted in state of practice.
• Valid driver’s license and reliable transportation.
• Understanding of EMR and HIPAA.
• Knowledge of community resources.
• Ability to work independently and manage multiple projects.

Responsibilities

• Perform comprehensive member assessments within timelines.
• Develop and implement care coordination plans with the care team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plans and adjust as needed.
• Maintain ongoing member caseload and outreach.
• Coordinate services across behavioral health, LTSS, and community resources.
• Lead ICT meetings and document interventions.
• Educate and motivate members using motivational interviewing.
