Job summary

Molina Healthcare is seeking a remote RN Care Manager to support Washington State Health Plan members through telephonic case management and transition-of-care activities. The role is remote with a PST schedule and optional preference for Washington state residents; no travel, no direct patient care, weekends, holidays, or night shifts.

Qualifications

• At least 2 years experience in health care, preferably in care management.
• RN with active, unrestricted license in state of practice.
• Valid driver’s license and reliable transportation for travel.
• Understanding of EMR and HIPAA.
• Demonstrated knowledge of community resources.
• Ability to operate proactively and detail-oriented work.
• Ability to work within varied settings with diverse populations.
• Ability to work independently with minimal supervision.

Responsibilities

• Complete comprehensive assessments of members per timelines and identify who may qualify for care management.
• Develop and implement care coordination plan with member and care team.
• Conduct telephonic, home visits or face-to-face meetings as required.
• Monitor care plan effectiveness and document interventions and outcomes.
• Maintain ongoing member caseload with regular outreach.
• Promote integration of behavioral health, LTSS, and community resources.
• Facilitate interdisciplinary care team meetings and collaboration.
• Educate and motivate members using motivational interviewing.
• Assess barriers to care and provide coordination assistance.
• May provide consultation and resources to peers as needed.
• Care managers may handle complex cases and medication reconciliation.
• Travel up to 25-40% may be required.
