Job summary

Point32Health is seeking a Care Manager - Nursing Field (RN CM) to ensure timely care management across the continuum, including transitions of care, care coordination, and population health initiatives. The RN CM will partner with members, caregivers, and providers to develop and implement individualized care plans and improve outcomes.

The role requires strong clinical knowledge, critical thinking, and the ability to empower members toward optimal wellness and self-management.

Qualifications

• RN with current unrestricted license in state of residence
• Bachelor's degree in nursing (required)
• National case management certification desirable

Responsibilities

• Perform telephonic or in-person member outreach to enroll in programs
• Provide targeted health education and proactive strategies for condition management
• Coordinate safe transitions of care and follow-up services
• Collaborate with interdisciplinary care team to improve member outcomes
• Maintain documentation in applicable platforms per policy and standards
