Job summary

Boston Medical Center seeks a Complex Care Manager RN to join the population health team. Based in primary care clinics, the role partners with PCPs, Integrated Behavioral Health, pharmacists, and community resources to develop multidisciplinar care plans for patients with chronic disease and social determinants influencing health outcomes.

The RN will conduct comprehensive assessments and coordinate care across transitions, supporting patient self-management and engagement in care decisions.

Qualifications

• Nursing degree (Diploma/ASN/BSN) required or equivalent.
• CCM certification preferred; RN licensure in MA or NP credentials.
• Minimum two years care management experience preferred.
• Ability to work with diverse, safety-net populations and community resources.

Responsibilities

• Identify and recruit appropriate patients for care management from lists and referrals.
• Execute core care management duties: comprehensive bio-psycho-social-spiritual assessments.
• Develop patient-centered care plans in collaboration with the care team.
• Coordinate with community partners and monitor goal attainment and transitions of care.
