Job summary

Point32Health, Inc. is looking for a skilled RN Care Manager (Nursing Field) to ensure timely care management across the continuum, including transitions of care, care coordination, and complex case management.

Qualifications

• RN with current unrestricted license in state of residence.
• National Case Management certification desirable.
• 5 years of clinical experience required.
• Home care or case management experience preferred.
• Second language proficiency desirable.

Responsibilities

• Perform telephonic member outreach and/or face-to-face encounters to enroll in programs and develop a member-specific plan of care.
• Provide targeted health education and proactive strategies for condition management.
• Coordinate safe transitions to the next level of care and ensure follow-up.
• Collaborate with interdisciplinary care team and present high-risk members as appropriate.
• Maintain professional growth through self-directed learning.
