Job summary

Point32Health is seeking a Care Manager – Nursing Field (RN CM) to ensure timely care management across the continuum, including transitions of care, care coordination, and disease management per department guidelines. • At least 5 years of relevant clinical experience; home care or case management preferred.
• Proficiency with Microsoft applications and care-management systems.

Responsibilities

• Perform telephonic member outreach and/or face-to-face encounters for program enrollment.
• Assess members to identify needs, health goals, and barriers to wellness.
• Develop and revise a member-specific plan of care with providers.
• Document in applicable platform per policy and regulatory standards.
• Provide targeted health education and proactive strategies for condition management.
• Coordinate safe transitions to the next level of care and post-discharge instructions.
• Collaborate with interdisciplinary care team to improve member outcomes.
• Present high-risk members at interdisciplinary rounds; pursue ongoing professional development.
