Qualifications

• RN licensure and nursing degree required.
• 2–4 years of related care management experience.
• Experience coordinating care and creating care plans with providers preferred.

Responsibilities

• Evaluate member needs and recommend plans for optimal outcomes.
• Develop ongoing care plans and collaborate with providers.
• Identify barriers to care and provide interventions.
• Coordinate between member and care team for person-centered care.
• Provide ongoing follow-up and monitoring of status and goals.
• Facilitate care management and ensure timely access to care.
• Conduct telephonic, digital, home, or other site outreach.
• Collect, document, and maintain member information for compliance.
• Educate members and families on disease processes and care options.
• Provide feedback to leadership to improve care delivery.
