Job summary

Centene Corp. is seeking a Senior Care Manager (RN) to assess, plan, and coordinate complex care for members, creating personalized care plans and educating families on services. The role involves up to 75% local travel to Wayne and Macomb counties, with the remainder of the time spent working remotely from home.

• Active RN license required.
• Bachelor's degree in Nursing or equivalent.
• 4–6 years of related care management experience.

Responsibilities

• Assesses, plans, and implements complex care management activities.
• Develops personalized care/service plans and educates members and families.
• Coordinates with providers to identify resources and services for member needs.
• Monitors care plans and adjusts as needed to meet goals.
• Travels up to 75% to support members in Wayne and Macomb counties; remote work the rest of the time.
