Centene Corporation is seeking a registered nurse to lead complex care management for primarily physical needs members. The role supports education for members and families, develops care plans, and coordinates with providers to improve health outcomes.

Remote work within Missouri is supported, with standard weekday hours. Ideal candidates hold an RN license and a nursing degree, with 2–4 years of related experience.

Formación

• Requires a nursing degree and 2–4 years of related experience.
• RN licensure in good standing for Missouri.
• Experience coordinating care across providers and settings is preferred.

Responsabilidades

• Evaluates member needs, barriers to access, and social determinants of health to plan outcomes.
• Develops care/service plans and collaborates with providers and community resources.
• Identifies problems and interventions to improve access and quality of care.
• Coordinates between members, families, and care teams to ensure person-centered care.
• Provides ongoing follow-up, monitors condition changes, and revises plans as needed.
• Offers resource support for local services (housing, employment, etc.).
• Facilitates timely access to needed care and services; may conduct telephonic or home outreach.
