Job summary

Centene Corporation is seeking a licensed Registered Nurse to develop and coordinate complex care management plans for members with physical health needs. The role emphasizes patient education, care coordination with providers, and ongoing follow-up to ensure high-quality, cost-effective outcomes.

The position requires active Kentucky state or compact RN licensure and typically operates Monday through Friday, with daytime hours in CT or ET.

Qualifications

• Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2–4 years of related experience.
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required

Responsibilities

• Evaluates the needs of the member and barriers to accessing care and recommends or facilitates the care plan.
• Develops ongoing care plans and collaborates with providers to identify resources.
• Identifies problems/barriers to care and provides care management interventions.
• Coordinates between member/family and care provider team to ensure person-centered care.
• Provides ongoing follow up and monitoring of member status and progress toward goals.
• Provides resource support to members and care managers for local services.
• Facilitate care management and collaborate with providers to ensure timely access to needed care.
• May perform telephonic, digital, home and/or other site outreach to assess member needs.
• Collects and maintains all member information to ensure compliance with regulations.
• Provides education to members and families on disease processes, care options, and benefits.
• Provides feedback to leadership on opportunities to improve care and quality delivery.
