Job summary

Hospice/home health experience is preferred, maternity experience is helpful, and MS Office proficiency is expected to support documentation and communication.

Qualifications

• RN licensure and nursing degree required.
• 2–4 years of related care management experience.
• Experience in disease management and education for patients and families.

Responsibilities

• Evaluate member needs and barriers to care, and plan outcomes.
• Develop and coordinate ongoing care/service plans with providers and resources.
• Identify problems and implement care management interventions.
• Coordinate between member/family and care team for person-centered care.
• Provide follow-up to monitor status and update care plans as needed.
• Offer education to members and families about disease processes and benefits.
