Humana Inc. and Inclusa seek a Registered Nurse Care Manager in Wisconsin to support Family Care members with care coordination, education, and documentation. The role offers no night or weekend shifts, remote work from home, and up to 40% travel to member sites.

You will assess health and safety needs, develop a comprehensive MCP, and coordinate with providers while ensuring timely record reviews within DHS timelines. You will guide new RNs and Care Coach partners in using the D365 system.

Qualifikationen

• Associate degree in nursing is required.
• Licensed Registered Nurse in the state of Wisconsin in good standing.

• Assess members and develop a comprehensive Member Care Plan (MCP).
• Conduct regular health assessments.
• Coordinate with acute and primary care providers.
• Educate members on diseases, nutrition, exercise and lifestyle changes.
• Document in member files within required DHS timelines.
• Maintain monthly member contacts and travel to in-person visits.
• Travel up to 40% to visit members.
• Review records for reassignment and remediation as needed.
• Notify managers of missed follow-ups and risks identified in reviews.
