Humana is seeking a RN Care Manager to support Wisconsin Family Care members with care coordination, education, and support. This role offers a true work-life balance with no night or weekend shifts, and involves remote work with travel up to 40% to visit members.

You will assess health and safety needs, develop MCPs, educate members on disease processes and healthy lifestyles, and coordinate with primary and acute care providers.

Qualifications

• Active Wisconsin RN license in good standing.
• Associate degree in nursing.
• Ability to travel up to 40% of the time.

Responsibilities

• Assess members and develop a comprehensive Member Care Plan (MCP).
• Conduct regular health assessments.
• Coordinate with acute and primary care providers.
• Educate members on disease processes, risks, nutrition, exercise and lifestyle modifications to support informed decision-making while respecting autonomy.
• Document in member files within required DHS contract timeframes.
• Participate in member monthly phone contacts and travel to attend in person visits.
• Complete record reviews for reassigned members and identify remediation needs.
• Ensure new RNs and Care Coach partners are informed of missing follow-ups and assign tasks in D365.
