Job summary

Humana is seeking a RN Care Manager to support Wisconsin Family Care members with care coordination, education, and documentation. This remote role requires travel up to 40% to member homes and clinics. Typical hours are 8:00 am–4:30 pm CST, Monday–Friday, with no night or weekend shifts.

You will collaborate with Care Coaches and providers to improve outcomes. Requires WI RN license and at least an associate degree in nursing; 3 years of care management is preferred.

Qualifications

• Associate degree in nursing required.
• Licensed Registered Nurse in Wisconsin in good standing.

Responsibilities

• Assess members and develop a comprehensive MCP focusing on health and safety.
• Conduct regular health assessments and coordinate with providers.
• Educate members on disease processes, nutrition, exercise and lifestyles.
• Document in member files within required DHS timeframes.
• Travel to member visits and complete remediations as needed.
• Ensure RNs and Care Coach are informed of follow-ups in D365.
• Support new member assignments and complex cases as needed.
