Humana's Inclusa division seeks a RN Care Manager to support Wisconsin Family Care members with care coordination, education, and connection to providers. The role is remote from home with travel about 40% to visit members in Lincoln, Langlade, Oneida, Forest and Vilas counties.

Requirements include an Associate degree in nursing and Wisconsin RN license; no night or weekend shifts; typical hours 8:00 to 4:30; pay range shown; benefits include medical, dental, 401(k).
