Job summary

Humana Inc. is seeking an RN Care Manager in Wisconsin to coordinate member care from home with about 40% travel. You will assess health and safety needs, develop MCPs, and educate members on health decisions while maintaining DHS timelines.

The role offers weekday hours and remote work with field visits as required. Requirements include an Associate degree in nursing and Wisconsin RN licensure; 1 year care management experience is preferred.

Qualifications

• Associate degree in nursing is required.
• Licensed Registered Nurse in Wisconsin (in good standing).
• 1 year Care Management experience (preferred).
• Reside within 45 mins or less of assigned coverage area (preferred).
• Previous Care Management experience (preferred).

Responsibilities

• Assess members to develop a comprehensive Member Care Plan (MCP).
• Conduct regular health assessments and coordinate with providers.
• Educate members on disease processes, nutrition, exercise and lifestyle changes.
• Document in member files within DHS timelines.
• Travel to members' settings and perform in-person visits as needed.
