Job summary

Humana, in partnership with Inclusa, is seeking a Nurse Care Manager to support Wisconsin Family Care members. This role offers a Monday–Friday schedule, remote work from home with up to 40% travel to member sites, and comprehensive care coordination, education, and record remediation within DHS timelines.

The ideal candidate holds an Associate Degree in Nursing and an active Wisconsin RN license, with 1 year of care management experience preferred and residence within 45 minutes of the coverage

Qualifications

• Associate degree in nursing required.
• Wisconsin RN license in good standing required.
• 1 year Care Management experience preferred.
• Residence within 45 minutes or less of the assigned coverage area preferred.

Responsibilities

• Assess members to develop a comprehensive Member Care Plan (MCP).
• Conduct regular health assessments.
• Coordinate with acute and primary care providers.
• Educate members on disease processes, risks, nutrition, exercise and lifestyle modifications.
• Document in member files within required DHS contract timeframes.
• Travel to visit members and conduct in-person visits as needed.
• Review records for reassignment and remediation; inform managers of risks or concerns.
