Job summary

Humana is seeking an RN Care Manager to support Wisconsin Family Care members with care coordination, education, and record remediation. This role offers a true work-life balance with no night or weekend shifts and remote work from home.

You will travel about 40% to visit members, assess health and safety needs, develop MCPs, educate on disease management, and coordinate with providers, documenting in DHS timelines.

Qualifications

• Associate degree in nursing (RN) with current Wisconsin license.
• 1 year Care Management experience preferred.
• Reside within 45 minutes or less of assigned coverage area.
• Must meet Humana TB screening and driver safety requirements.

Responsibilities

• Assess members to develop a comprehensive MCP.
• Conduct regular health assessments.
• Coordinate with acute and primary care providers.
• Educate members on disease processes, nutrition, exercise and lifestyle changes.
• Document in member files within required DHS contract timeframes.
• Participate in monthly member phone contacts and travel for in-person visits.
• Travel approximately 40% to visit members.
• Complete record reviews and remediation as needed.
• Inform Managers of significant risks during record reviews.
• Support onboarding and assignments for new members.
