Job summary

Humana Inc. is seeking a registered nurse to serve as RN Care Manager for Wisconsin Family Care members. You will work remotely with up to 40% travel, coordinating care, educating members, and supporting care coaches under the Wisconsin program.

Responsibilities include creating MCPs, conducting health assessments, and collaborating with providers to improve health outcomes while respecting autonomy.

Qualifications

• Associate degree in nursing required.
• Licensed Registered Nurse in Wisconsin in good standing.

Responsibilities

• Assess members and develop a comprehensive Member Care Plan (MCP).
• Conduct regular health assessments.
• Coordinate with acute and primary care providers.
• Educate members on disease processes, risks, nutrition, exercise, and lifestyle changes.
• Document in member files within DHS timeframes.
• Travel to member visits and perform monthly phone contacts.
