Job summary

Humana Inc. is hiring a RN Care Manager to coordinate care for Wisconsin Family Care members. You will conduct health assessments, develop MCPs, educate members, and document within DHS timelines while working remotely from home with about 40% travel to client sites in Hayward and Spooner region.

This role requires Wisconsin RN licensure, an associate degree in nursing, and preferred care management experience; typical hours are Mon-Fri, 8:00 am–4:30 pm CST with travel up to 40%.

Qualifications

• Associate degree in nursing is required.
• RN license in Wisconsin in good standing.

Responsibilities

• Assess members to develop a comprehensive Monthly Care Plan (MCP).
• Conduct regular health assessments.
• Coordinate with acute and primary care providers.
• Educate members on disease processes, nutrition, exercise and lifestyle changes.
• Document in member files within required DHS timelines.
• Travel approximately 40% to visit members; work remotely from home.
