Job summary

Humana Inc. seeks an RN Care Manager to support Wisconsin Family Care members, providing care coordination, education, and support. This remote role requires ~40% travel to member sites and daily coordination with care teams in La Crosse and surrounding areas.

Qualified candidates hold an Associate degree in nursing and Wisconsin RN licensure. Typical hours are M–F, 8:00 am–4:30 pm, with benefits and a competitive salary range.

Qualifications

• 3 years Care Management experience
• Reside within 45 mins or less of the assigned coverage area
• Previous Care Management experience

Responsibilities

• Assess members, focusing on health and safety needs, to develop a comprehensive Member Care Plan (MCP).
• Conduct regular health assessments and coordinate with acute and primary care providers.
• Educate members on disease processes, risks, nutrition, exercise and lifestyle modifications.
• Document in member files within required DHS contract timeframes.
• Participate in member monthly phone contacts and travel for in-person visits.
• Complete record reviews for reassigned members and identify remediation needs.
