Job summary

Humana is seeking an experienced Registered Nurse Care Manager to support Wisconsin Family Care program members. This remote role offers a Monday–Friday schedule (8:00 am–4:30 pm CST) with no nights or weekends and travel up to 40%.

You will develop MCPs, perform health assessments, and educate members on disease processes and healthy lifestyles. The position requires an RN license in Wisconsin, at least an Associate degree, and prior care management experience is preferred.

Qualifications

• Associate degree in nursing is required.
• 1 year care management experience preferred.
• Licensed Registered Nurse in Wisconsin in good standing.

Responsibilities

• Assess members to develop a comprehensive Member Care Plan (MCP).
• Conduct regular health assessments.
• Coordinate with acute and primary care providers.
• Educate members on disease processes, nutrition, and lifestyle modifications.
• Document in member files within required DHS contract timeframes.
• Participate in member monthly phone contacts and travel for in-person visits.
• Complete record reviews for reassigned members.
• Ensure newly assigned RNs and Care Coach partners are informed of follow-up tasks.
• Provide support regarding record review and remediation.
• Accept additional complex member assignments.
