Qualifications

• Associate degree in nursing and Wisconsin RN license in good standing.
• Reside within 45 minutes or less of the assigned coverage area.
• 1 year care management experience preferred.

Responsibilities

• Assess members to develop a comprehensive Member Care Plan (MCP).
• Conduct regular health assessments and coordinate with providers.
• Educate members on disease processes, risks, nutrition and lifestyle choices.
• Document in member files within required DHS timeframes.
• Perform monthly phone contacts and in-person visits as needed.
• Travel up to 40% to visit members; work from home when not traveling.
