Job summary

Optum seeks an RN Care Coordinator to join the Insight ACO Practice Extend Team. You will work with a clinical pharmacist and administrative coordinator to manage high-risk patients and coordinate care transitions.

You will perform telephonic outreach, educate patients, and coordinate with PCPs, specialists, and care teams to ensure high-quality, cost-effective care delivery. Remote within the U.S. scope.

Qualifications

• Bachelor's degree required.
• Active RN license in a compact state within the US.
• 3+ years of Case Management/Care Coordination experience.
• Advanced level of proficiency with Microsoft Office (Excel and PowerPoint).
• Ability to work 1 late shift per week (10am–7pm).

Responsibilities

• Support patient and provider engagement across care transitions, care coordination, and medication management activities.
• Communicate discharge information to primary care offices using appropriate technology tools.
• Post-discharge outreach including medication reviews and timely follow-up appointments.
• Guide patients to high-quality, cost-effective referrals.
• Coordinate appointments for high-risk patients with PCPs or specialists.
• Provide patients with benefit information and resources on request.
• Coordinate with practice staff and care team to resolve care issues.
• Maintain relationships with participating provider practices and communicate program goals.
• Lead provider-facing meetings and onboarding sessions.
