Job summary

UnitedHealth Group is seeking an RN Care Coordinator to join the Optum Insight ACO Practice Extend Team. You will collaborate with a Clinical Pharmacist and Administrative Coordinator to improve health outcomes, focusing on care transitions, risk management, referrals, and appropriate site-of-service utilization.

This role offers telecommuting from anywhere in the U.S. and involves high-volume outreach, patient education, and coordination with PCPs and specialists.

Qualifications

• Bachelor's degree required
• Current, active, unrestricted RN license in a Compact State within the US
• 3+ years in Case Management/Care Coordination
• Advanced proficiency in Microsoft Office, especially Excel and PowerPoint
• Advanced typing skills and Windows-based navigation
• Ability to work 1 late shift per week (10am-7pm)

Responsibilities

• Support patient and provider engagement across care transitions and medications management
• Facilitate inpatient discharge communications to primary care offices
• Conduct post-discharge outreach and schedule timely follow ups
• Guide patients to high-quality, cost-effective referrals
• Coordinate high-risk patient appointments with PCPs or specialists
• Provide patients with benefit information and resources
• Communicate with provider offices to resolve care issues
• Build relationships with participating practices and communicate program goals
• Lead provider-facing and onboarding meetings
