You will partner with a Clinical Pharmacist and Administrative Coordinator to manage care transitions, high-risk patients, and referral coordination across provider practices, ensuring high-quality care delivery.

Qualifications

• Bachelor's degree required.
• Current, active, unrestricted RN license in a Compact State within the US.
• 3+ years of case management or care coordination experience.
• Advanced proficiency in Microsoft Office (Excel/PowerPoint).
• Ability to type and navigate Windows-based systems efficiently.

Responsibilities

• Collaborate with a Care Team to support patient and provider engagement across transitions, coordination, and medication management.
• Discharge communications to primary care offices using appropriate tools.
• Conduct post-discharge outreach including follow-up appointments and education on discharge instructions.
• Coordinate high-risk patients with PCP and/or specialist appointments.
• Provide patients with benefit information and resources on request.
• Communicate with providers to resolve care issues and support team collaboration.
• Build relationships with participating practices and communicate program goals.
• Lead provider-facing meetings and ongoing collaboration sessions.
