Job summary

UnitedHealth Group is seeking an RN Care Coordinator to join the Optum Insight ACO Practice Extend Team. You will work with a Clinical Pharmacist and Administrative Coordinator to improve outcomes for patients in provider practices participating in the Optum Insight ACO.

Responsibilities include care transitions, high-risk patient management, referral coordination, and appropriate site-of-service utilization, with high-volume telephonic outreach and patient education.

Qualifications

• Bachelor's degree required.
• Current, active, unrestricted RN license in a Compact License state within the US.
• 3+ years of experience in Case Management or Care Coordination.
• Advanced proficiency in Microsoft Office, especially Excel and PowerPoint.

Responsibilities

• Support patient and provider engagement across care transitions, care coordination, and medication management.
• Facilitate communication of discharge information to primary care offices.
• Conduct post-discharge outreach including scheduling follow ups and educating on discharge instructions.
• Coordinate high-quality, cost-effective specialist referrals and appointments.
• Provide benefit information and resources to patients and coordinate care with PCPs and specialists.
• Build strong relationships with participating practices and lead provider-facing meetings.
