Job summary

UnitedHealth Group is seeking an RN Care Coordinator for the Optum Insight ACO Practice Extend Team. You will work with a Clinical Pharmacist and Clinical Administrative Coordinator to improve patient outcomes, focusing on care transitions, high-risk patients, and referral coordination.

This role includes significant telephonic outreach, patient education, and coordination with PCPs, specialists, and the care team to ensure high-quality care delivery.

Qualifications

• Bachelor's degree required
• Current, active, unrestricted Registered Nurse (RN) license in a compact license state within the US
• 3+ years of experience in Case Management and/or Care Coordination
• Advanced proficiency in Microsoft Office, especially Excel and PowerPoint
• Advanced ability to type and navigate a Windows-based environment
• Ability to work 1 late shift per week (10am-7pm) depending on time zone business hours

Responsibilities

• Support patient and provider engagement across care transitions management, care coordination, and medication management
• Facilitate communication of discharge information to primary care offices
• Conduct post-discharge outreach, medication reviews, and scheduling follow-ups
• Guide patients to high-quality, cost-effective specialist referrals
• Coordinate and schedule PCP and/or specialist appointments for high-risk patients
• Provide patients with benefit information and resources
• Communicate with provider offices to resolve patient care issues
• Build relationships with participating provider practices and communicate program goals
• Lead provider-facing onboarding and ongoing collaboration meetings
