You will collaborate with physicians, nurses, pharmacists, social workers and care teams to educate, advocate, and coordinate transitions of care, aiming to reduce hospitalizations and improve quality outcomes.

Qualifications

• Current NV RN license is required.
• BLS certification or willingness to obtain within 3 months.
• 2+ years of experience in an acute care setting.
• Knowledge of medical terminology; nursing assessment and follow-up.
• Proficiency with EMR and MS Office applications.

Responsibilities

• Role embedded within the primary care clinic, coordinating with clinical and non-clinical teams.
• Assess health conditions and collaborate with patient, caregiver, and clinicians to set goals.
• Implement interventions to reduce risk of exacerbation and ED visits.
• Facilitate transition of care from ER or inpatient to outpatient settings; arrange referrals.
• Contribute to achieving Quality Measures and HEDIS outcomes.
