Job summary

Optum is seeking an RN Care Coordinator to join the Insight ACO Practice Extend Team. You will work with a Clinical Pharmacist and Administrative Coordinator to improve patient outcomes, focusing on care transitions and high‑risk patient management. The position offers telecommuting from anywhere in the U.S.

and a flexible schedule aligned with Central/Mountain/Arizona time zones. Responsibilities include discharge communication, post‑discharge outreach, scheduling, and education on care sites.

Qualifications

• Bachelor's degree required.
• Current, active, unrestricted RN license in a Compact License state within the US.
• 3+ years of experience in Case Management and/or Care Coordination.
• Advanced proficiency in Microsoft Office, particularly Excel and PowerPoint.
• Ability to work 1 late shift per week (10am-7pm) with times reflecting the assigned time zone.

Responsibilities

• Collaborate with the Care Team to support patient and provider engagement across care transitions, coordination, and medication management.
• Facilitate discharge communications to primary care offices using appropriate technology tools.
• Conduct post-discharge outreach, medication reviews, scheduling follow-ups, and patient education on discharge instructions.
• Guide patients in accessing high-quality, cost-effective specialist referrals.
• Coordinate and schedule PCP and/or specialist appointments for high-risk patients.
• Provide patients with benefit information and resources on request.
• Communicate with provider offices to resolve care issues and improve care team collaboration.
• Build strong relationships with participating practices and communicate program goals and value-based care opportunities.
• Lead provider-facing meetings, onboarding, and ongoing collaboration sessions.
