Job summary

UnitedHealth Group seeks an RN Care Coordinator to join the Optum Insight ACO Practice Extend Team. You will manage care transitions, coordinate referrals, and educate patients across high-volume telephonic outreach. Work flexibly from anywhere in the U.S. to support clinician teams and improve outcomes.

Your role emphasizes discharge communication, medication reviews, and coordinating with PCPs and specialists to ensure high-quality, cost-effective care delivery.

Qualifications

• Bachelor's degree required.
• Active RN license in a Compact State within the US.
• 3+ years in Case Management and/or Care Coordination.
• Advanced proficiency in Microsoft Office (Excel and PowerPoint).
• High typing speed and ability to navigate Windows in a fast-paced environment.
• Ability to work one late shift per week (10am-7pm) in appropriate TZ.

Responsibilities

• Work with a Care Team to support patient and provider engagement across transitions, coordination, and medication management.
• Facilitate communication of discharge information to primary care offices using suitable tools.
• Conduct post-discharge outreach for inpatient/ED visits, med reviews, follow-ups, and education.
• Guide patients in accessing high-quality, cost-effective referrals.
• Coordinate and schedule PCP and/or specialist appointments for high-risk patients.
• Provide patients with benefits information and resources as requested.
• Communicate with provider offices to resolve care issues and support team collaboration.
• Build relationships with practices and communicate program goals and value-based care opportunities.
• Facilitate or lead provider-facing onboarding and collaboration meetings.
