Job summary

UnitedHealth Group, through Optum, seeks an RN Care Coordinator to join the Insight ACO Practice Extend Team. The role focuses on care transitions, high-risk patient management, and referral coordination with telephonic outreach across US-based provider offices.

The position emphasizes collaboration with a clinical pharmacist and administrative coordinator to improve patient outcomes and ensure high-quality care delivery.

Qualifications

• Bachelor's degree in nursing required.
• Active RN license in a Compact License state in the US.
• 3+ years in Case Management and/or Care Coordination.
• Proficient in Microsoft Office (Excel/PowerPoint).

Responsibilities

• Support patient and provider engagement across care transitions and medication management.
• Discharge communications to primary care offices using appropriate tools.
• Post-discharge outreach including medication reviews and follow-up scheduling.
• Coordinate high-risk patient appointments with PCPs and specialists.
• Provide patients with benefit information and resources.
• Facilitate provider meetings and ongoing collaboration with the care team.
• Maintain relationships with participating practices and communicate program goals.
