Job summary

UnitedHealth Group is seeking a Care Manager, RN, to lead patient-centered care coordination across the continuum. The role focuses on discharge planning, LOS optimization, and collaboration with a multidisciplinary team to improve patient outcomes and reimbursement.

You will work remotely from anywhere in the U.S., with the flexibility to support diverse patient populations and care settings, guided by evidence-based practices and regulatory compliance.

Qualifications

• Associate's degree in Nursing (RN required).
• Current, unrestricted RN license in the state of residence.
• 3+ years of experience in a hospital, acute care, or direct care setting.
• Intermediate proficiency to type and navigate a Windows-based environment.

Responsibilities

• Coordinate patient care processes to achieve desired quality outcomes.
• Directly communicate with staff, physicians, patients, and families.
• Lead interdisciplinary collaboration and discharge planning to ensure smooth transitions.
• Facilitate patient education and promote continuity of care.
• Adhere to CMS and Joint Commission standards and ensure timely documentation.
