Job summary

Sagility is seeking an experienced RN for a Seasonal Senior Care Specialist role, working from home in the United States. You will assess members by phone, develop care plans, provide education, and coordinate services to improve health outcomes for Medicare/Medicaid groups.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking and problem-solving skills.
• Ability to work independently in a remote environment.
• Proficiency with Microsoft Word, Excel, and Outlook.
• Ability to navigate multiple computer systems and electronic medical records.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on needs and health risks.
• Establish health coaching relationships and provide education and support for chronic disease management.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Coordinate transitions of care and arrange home health services and DME.
• Assist members in navigating healthcare benefits and services.
• Document interactions, assessments, care plans, and outcomes accurately in systems.
