Sagility is seeking a Senior Care Specialist (Registered Nurse) to deliver remote care coordination and telephonic health assessments for Medicare, Medicaid, and group health plan members. Qualifikationen

• Active, unrestricted Registered Nurse license in the state of residence.
• Compact RN license allowing practice in applicable compact states.
• Minimum of 2 years clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking and problem-solving skills.
• Ability to work independently and exercise nursing judgment remotely.
• Intermediate to advanced computer proficiency including Word, Excel, and Outlook.

Aufgaben

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and risks.
• Establish health coaching relationships and provide education, encouragement, and support for chronic condition management.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including DME and home health services.
• Help members navigate healthcare benefits, services, and care options.
