Job summary

Sagility is seeking an experienced Senior Care Specialist (Registered Nurse) for a remote, work-from-home seasonal role in the United States. You will conduct telephonic health assessments, develop individualized care plans, and coach members on chronic condition management while coordinating with a multidisciplinary team.

This role requires an active RN license (state of residence) and a Compact licensure, with 2+ years of nursing experience.

Qualifications

• Active, unrestricted RN license in state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, problem-solving skills.
• Ability to work independently and in remote environment.
• Intermediate to advanced computer proficiency (Word, Excel, Outlook).
• Ability to navigate multiple EMR systems.
• Strong verbal and written communication skills.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and health risks.
• Establish health coaching relationships with members and provide education and support for chronic conditions.
• Use clinical nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including DME coordination.
• Help members navigate benefits, services, and care options.
• Assess factors that affect health and well-being.
• Collaborate with multidisciplinary teams.
• Promote member self-management with education and resources.
• Coordinate care and condition-management activities among care team.
• Document interactions, assessments, plans, interventions, and outcomes accurately.
