Job summary

Sagility is seeking experienced RNs to join as Senior Care Specialists in a work-from-home capacity. This seasonal role runs through June 2027, with potential bonuses based on performance and business needs.

You'll assess members by phone, develop care plans, educate patients, and coordinate services to improve outcomes. Qualified candidates will have an active RN license in their state, a Compact license, and 2+ years of nursing experience.

Qualifications

• Active, unrestricted RN license in state of residence.
• Active Compact RN license permitting practice in applicable states.
• Minimum of 2 years clinical nursing experience (3+ preferred).
• Ability to work independently in a remote environment.
• Intermediate to advanced computer proficiency (Word, Excel, Outlook).
• Ability to navigate multiple EMR systems.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop individualized, measurable care plans based on identified health risks.
• Provide health coaching, education, and support for chronic condition management.
• Coordinate transitions of care and discharge planning with DME, home health, and providers.
• Identify barriers to care and promote improved health outcomes with a Whole Person Care approach.
• Collaborate with multidisciplinary teams to support member health goals.
• Document interactions, assessments, and outcomes timely in systems.
