Zusammenfassung

Sagility is seeking experienced Registered Nurses to join as Senior Care Specialists in a work-from-home capacity for a seasonal assignment ending in June 2027. You will assess members by phone, develop care plans, educate patients, and coordinate services to improve health outcomes.

Qualified candidates will have an active RN license, Compact RN eligibility, 2+ years of nursing experience, and strong communication and technical skills.

Qualifikationen

Active, unrestricted RN license in state of residence.
Active Compact RN license permitting practice in applicable compact states.
Minimum 2 years of clinical nursing experience; 3+ years preferred.
Remote work ability.
MS Word, Excel, and Outlook proficiency.
Ability to navigate multiple EMR systems.

Aufgaben

Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
Develop individualized, measurable care plans based on member needs and health risks.
Establish health coaching relationships and provide education and support for chronic condition management.
Coordinate transitions of care and arrange DME, home health services, and provider appointments.
Document interactions, assessments, care plans, and outcomes accurately in systems.
