Job summary

Color Health, Inc is seeking a dedicated RN to join our Virtual Cancer Clinic care team. This role supports patients with symptoms, provides education, and coordinates timely follow-up after abnormal screenings, in collaboration with physicians, genetic counselors, and care advocates.

The ideal candidate has a BSN, an active RN license (NLC preferred), and at least 7 years of nursing, including 5 in primary care.

Qualifications

• BSN required and active RN license.
• NLC multi-state license preferred.
• 7+ years nursing experience, 5+ years in primary care.
• Outpatient experience required; oncology a plus.

Responsibilities

• Triage, care coordination, and patient education via phone and async messaging.
• Conduct clinical assessments and follow-up for abnormal screening results.
• Coordinate follow-up care and referrals with care team.
• Maintain thorough documentation of patient encounters.
