Job summary

Color is seeking a dedicated Registered Nurse to join its Virtual Cancer Clinic team. In a telehealth capacity, you will triage, educate, and coordinate care for patients across the United States, working with physicians, NP/PA providers, and care advocates.

The ideal candidate holds a BSN, active RN license, and at least 7 years of nursing experience, including 5 in primary care. Bilingual English/Spanish is highly preferred, and the role follows Pacific Time business hours.

Qualifications

• Active RN license in good standing.
• Experience coordinating care via telehealth.
• Experience in outpatient/primary care, with follow-up.
• Ability to educate patients on screenings and results.
• Bilingual English/Spanish strongly preferred.

Responsibilities

• Serve as the clinical resource for cancer screening and prevention population via triage, care coordination, and education by phone and messaging.
• Perform needs assessments to identify patient needs and coordinate follow-up care.
• Conduct telephone visits for symptom management and follow-up, escalating when needed.
• Educate patients on abnormal screening results and next steps, arranging the next level of care.
• Collaborate with Care Advocates to address non-clinical support and logistics.
• Document encounters thoroughly and maintain continuity of care across the team.
