Qualifications

• Bachelor’s degree in public health, health administration, public policy, business administration, or related field
• 5 years of experience in grant management, contract oversight, program administration, or related work
• 3 years of experience working with healthcare organizations, rural communities, or public health systems
• 2 years of experience with compliance, budgeting, and reporting requirements for state or federal grants
• Ability to manage multiple projects simultaneously with strong attention to detail
• Excellent communication, facilitation, and relationship-building skills
• Ability to work independently as a remote field-based contractor
• Must reside within the assigned RHTP region and be willing to travel regularly within the region
• Experience with CMS-funded programs or federal cooperative agreements
• Familiarity with Indiana's rural health landscape, including hospitals, FQHCs, EMS, and community-based organizations
• Experience collaborating with statewide associations or multi-stakeholder coalitions
• Knowledge of rural health delivery, care coordination, or health system transformation

Responsibilities

• Serve as the primary regional contract manager for organizations receiving RHTP funding
• Monitor compliance with state and federal requirements, including expenditures, budget adherence, and documentation standards
• Support IDOH in ensuring regional coalitions meet milestones, deliverables, and timelines
• Identify risks and escalation to IDOH as appropriate
• Maintain audit-ready documentation and templates
• Collaborate with statewide Technical Assistance Providers to ensure aligned support
• Participate in regional meetings and convergence sessions
• Coordinate regional data collection and CMS reporting requirements
• Ensure timely submission of reports to IDOH
• Support regional partners in understanding CMS reporting expectations
• Serve as the liaison between IDOH and regional coalitions
• Promote representation of rural populations and local needs
• Maintain communication with IDOH’s Regional Grant Initiative Team
• Track regional progress and help solve barriers
• Use shared tools and evaluation frameworks
• Attend trainings and statewide meetings
