Job summary

Kintegra Health seeks a Practice Transformation Coordinator to promote quality improvement and support practice staff in EMR education and regulatory workflows. The role collaborates with clinicians, educators, and staff to drive health outcomes and patient experiences.

The coordinator will assist with QI initiatives, performance metrics, and system workflows, leveraging health IT to support compliant, patient-centered care delivery in a remote setting with substantial travel.

Qualifications

• Registered or Certified Medical Assistant or Licensed Practical Nurse.
• 3-5 years of clinical practice in medical office workflow and EMR documentation.

Responsibilities

• Assist in quality improvement methodologies to promote organizational change.
• Maintain knowledge of regulatory programs including Meaningful Use, UDS, ACO, PCMH, and HEDIS.
• Assist with departmental QI initiatives: identify metrics, interventions, evaluation, outcomes.
• Assist with report generation and evaluation.
• Review patient records and care gap reports for accuracy.
