Praxis Health is seeking a Quality Improvement Manager to lead and advance our quality performance with a focus on HEDIS, payer incentive programs, and population health initiatives. You will oversee identifying opportunities, monitor performance, and drive improvement strategies across a multi-clinic system.

You will partner with clinical and operational leaders, serve as EMR quality SME, optimize workflows, and develop staff education on quality requirements, documentation, and care gap

Qualifications

• Bachelor’s degree in healthcare admin, nursing, public health, or related field preferred.
• 3–5+ years in healthcare quality, HEDIS, population health, payer programs, or quality improvement.
• Experience managing or supervising a team.
• Strong knowledge of HEDIS measures, quality metrics, care gaps, and payer programs.
• Experience with EMR systems and quality-related reporting, Cerner/Oracle Health preferred.

Responsabilités

• Lead HEDIS performance strategy, monitoring, gap identification and improvement initiatives.
• Track and analyze performance across HEDIS measures to identify trends and opportunities.
• Develop interventions to close care gaps and improve clinical outcomes.
• Monitor progress toward quality goals for the organization, payers, and providers.
• Align quality initiatives with regulatory requirements and payer expectations.
• Translate quality data into actionable insights for clinical and operational teams.
• Lead and develop a team supporting HEDIS, quality improvement and payer incentives.
• Educate staff on HEDIS, quality requirements, payer programs, and care gap closure.
• Promote organizational awareness and accountability for quality performance.
