Job summary

Aledade PBC is seeking a Practice Transformation Specialist to support New England practices remotely with periodic on-site visits in Vermont and Massachusetts. This role focuses on implementing ACO initiatives, training staff, and driving improvements in population health and cost reduction.

The ideal candidate brings experience with EHR systems and strong communication, with travel roughly 1x per month to practices in the region.

Qualifications

• Must be resident of New England; 4-6 years of medical practice management experience may be accepted in lieu of formal education.
• 3 years of experience with Electronic Health Records (EHR) for clinical/practice management processes.

Responsibilities

• Direct Practice Support: Implement products and execute ACO initiatives with guidance from Market Leadership; ensure practices meet goals in Practice Transformation Plans.
• ACO Training & Development: Train office staff and providers on processes and technologies; assess training needs; build relationships to support performance.
• Customer Success: Listen to practices concerns and relay information to identify opportunities for improvement.
