Job summary

Aledade PBC is seeking a Practice Transformation Specialist to support New England practices with population health initiatives and cost management. The role blends remote collaboration with monthly on-site check-ins across Vermont and Massachusetts.

You will train staff, implement ACO initiatives, and build lasting relationships with physicians and healthcare executives to improve quality and financial performance while traveling as needed.

Qualifications

• Must be resident of New England.
• 4–6 years of significant experience in medical practice management; degrees may be substituted with administrative clinic experience.
• 3 years of experience with Electronic Health Records (EHR) for clinical/practice management processes.

Responsibilities

• Direct Practice Support: implement products and execute ACO initiatives for practices.
• Provide training to staff and physicians on processes and technologies.
• Develop relationships with physicians, providers, and healthcare executives to improve performance.
• Listen to practices’ concerns and identify opportunities for improvement.
