Job summary

Alliance Health seeks a Practice Transformation Specialist to lead quality improvement initiatives with advanced medical homes and care management agencies across the Alliance region. You will guide data-informed care, staff training, and CQI activities while coordinating with CINs to advance value-based care models.

The role emphasizes remote work with occasional Alliance Office meetings and requires strong analytic, training, and project-management skills to drive member outcomes and system

Qualifications

• Bachelor’s degree in nursing, social work, or other human services related field and five years of post-degree experience in healthcare or managed care.
• Experience leading healthcare quality and process improvement initiatives in an operational setting, particularly within community health.
• Experience in public speaking, mediation, presentation development, training, group facilitation, advocacy, regulatory compliance, and project management is preferred.

Responsibilities

• Provide support to network providers by evolving administrative and clinical workflows and processes to improve outcome-based care.
• Educate practice staff and CINs in CQI principles, Plan-Do-Study-Act cycles, and data-driven care.
• Review performance reports with providers and CINs, presenting data in an understandable format.
• Collaborate with AMHs and CMAs to integrate administrative, financial, and clinical data for better outcomes.
• Facilitate adoption of 1915(i) processes and HR.1 impacts for member care.
• Deliver practice-level education, training, and toolkits for improving member care and 1915 I/HR.1 requirements.
• Provide coaching at clinical sites using data for CM and value-based contracting.
